Medical Clearance and Preoperative Testing
Patients younger than 50 generally may not need a separate medical clearance from a primary care physician unless it is requested because of their medical history, symptoms, medications, planned procedure, anesthesia requirements, or hospital protocol.
Patients age 50 and older are generally required to obtain medical clearance from their primary care physician and complete the following preoperative evaluation:

CMP — Comprehensive Metabolic Panel
PT, PTT, and INR — coagulation studies
Urinalysis
Urine culture and sensitivity (Urine C&S)
Urine hCG pregnancy test, when applicable
